Client Identification Forms
The Law Society of Upper Canada requires lawyers in Ontario to maintain identification of all clients. Linked here are our standard Client ID Forms for individual clients, clients that are corporations or other organization, and clients that are condominium corporations. Copies of legal identification documentation, such as a valid Ontario Driver’s License, will also be required for each individual who might give us instructions, particularly with respect to funds. It is a considered a breach of our professional obligations to provide services to any client without obtaining this information. We provide our assurance that all personal information given to us for this purpose will be kept in strictest confidence and will not be used for any purpose other than satisfaction of the Law Society’s requirements.

Condominium Lien Instruction Form

Use this form to provide instructions to send a Notice of Lien and/or to Register a Certificate of Lien. This form covers the basic information needed to commence work on a condominium lien. If work relating to a condominium lien is requested without this form, we may request completion of the form or other provision of the information requested therein. It is also still available to you to email us instructions using the pdf form, which you can also find at the link above. |
Section 98 Agreement, Unit Owner Information FormThis form is used to provide personal information required for the proper preparation and registration of an agreement under section 98 of the Condominium Act, 1998. This information helps to ensure we can more efficiently and accurately prepare the form of agreement for unit owner execution. Our privacy policy in respect of such information is set out on the form.
